Birth & Death Certificates

PRICING
Certified copies of birth and death certificates are $25.00 each. We accept payment via cash, money order, or credit card. There is a 2.35% transaction fee for credit card purchases ($1.95 minimum). Identification matching credit card used is required. WE DO NOT ACCEPT PERSONAL CHECKS.  

REFUND POLICY


BIRTH CERTIFICATES (1909-Present)
Birth certificates are issued statewide across the state in which you were born.  If you were born in Ohio, you can go to your nearest Ohio health department to obtain a certified copy of your birth certificate.  If you were born in a different state, you will have to contact a health department in that state to obtain a certified copy.

To obtain a birth certificate from Butler County General Health District, you can do one of the following:

IN PERSON - 5 to 10 minutes (FASTEST)
Visit our office and fill out an application, and a certified copy will be printed for you within 5-10 minutes.  Please note: wait times might be longer between 11 AM and  1 PM due to high volumes.

ONLINE - 3 to 10 business days
Visit www.vitalchek.com to order a certified copy ($7.00 processing fee applies).  Your birth certificate will be sent via regular mail, or you can pay an extra fee for express shipping.


OVER THE PHONE 1 to 10 business days
Butler County General Health District does not currently accept birth certificate orders over the phone. You can call the City of Hamilton Health Department at (513) 785-7080 to place an order over the phone using your credit card, which can be picked up in person at 345 High Street, or mailed to you via regular mail.


DEATH CERTIFICATES (1909-Present)
Death certificates must be obtained from the health department that governs the city/township in which the decedent passed away.  Please see below if the decedent passed within Hamilton or Middletown city limits.

Effective 10/15/2015:
For death certificates of decedents that passed within the past 5 years, you are required to submit ID in order to have the death certificate include the social security number.  Only a lateral relative (parent, sibling, child, grandparent, etc.) of the decedent can obtain a copy of a death certificate that includes the social security number.  Please call us at (513) 863-1770 with any questions.
